This form satisfies the requirements in providing the Federal Consumer Credit and Insurance Disclosures. 2 inch stub containing the consumer disclosures can be easily detached and serves as the customer's copy. Imprinting is available on the reverse side of the disclosure. Form is available in 2-color (yellow and black) and is image-friendly.
